Please confirm that:

1. You have taken express permission (digitally or physically signed) from the customer / guardian to fill this form / questionnaire required for the purpose of applying for a scheme / document.
2. You have explained to the customer / guardian that we may use their details to design and offer more services for them, contacting them through phone, WhatsApp, SMS for business purposes such as verification, offers, surveys etc. Such explanation / notice must be provided in clear and plain language that is understood by the customer/ guardian.
3. You have explained to the customer / guardian that we do not share data with anyone but may share their details with our corporate partners who are sponsoring the service for them or with our evaluation partners for monitoring & evaluation purposes.
4. You have explained to the customer / guardian that we will store only the last four digits of their Aadhar number.
5. You have also informed the customer / guardian  that the consent given by them can be revoked by them any time in future and if so done, the Company will delete their personal data or ensure that such data is no longer in the nature of ‘personal data’ (that is, anonymise it).
